Our long term care home is hiring a skilled Occupational Therapist to join our team. Reporting to the Executive Director, you will play a key role in the assessment, treatment and prevention of functional decline for our residents. Through goals-based programs and person-centred care, you foster a sense of fulfilment and personal growth in the lives of others. This role includes: • Integrating client-centred care within a clinical practice; • Evaluating client needs on a case-by-case basis, using standardized assessment tools, best practices and clinical expertise; • Designing client-centred treatment plans, and a roadmap to success that is measurable, clear and attainable; • Helping families, caregivers and community partners understand the nature of your clients’ needs, and the reasoning behind your assessment; • Determining the need for additional services, cost-effective intervention strategies and discharge planning details; • Maintaining client-care records.
